VALID ID REQUIREMENTS

The ONLY acceptable forms of identification must be:

  • ORIGINAL

  • NON-EXPIRED

  • or ISSUED WITHIN THE LAST 5 YEARS

    NOTE: Valid ID must have the EXACT name or LONGER than the name as it appears on the document.

  • Original identification card OR driver’s license issued by California DMV

  • Original passport or passport card issued by Department of State of the United States

Forms of ID

  • Passport issued by a foreign government

  • Driver’s license issued by another state

  • Driver’s license issued by a Canadian or Mexican public agency

  • Identification card issued by another state

  • Identification card issued by a federally-recognized tribal government

The following must be original, non-expired, or issued within the last five years, AND must contain a photograph or description of the person named on it, have a signature by the person named on it, and bear a serial or identifying number. NOTE: Valid ID must have the EXACT name or LONGER than the name as it appears on the document.

Additional Forms of ID

  • Employee identification card issued by an agency of office of the State of California or city or county in the State of California

  • Consular ID - issued to immigrants by the consulates of their home country

  • Jails - Inmate identification card issued by the Department of Corrections and Rehabilitation

Credible witnesses may be used when the signer does not possess an acceptable form of identification. Two credible witnesses known to the signer will need to possess a form of ID as listed above. Any questions regarding credible witnesses, please contact us.
